Our Responsibility
Your contract with Ivy Health Surgery Limited, trading as Ivy Health Surgery, is limited to providing you with arrangement services pre-surgery for your trip to the SurGal Clinic in Brno Czech Republic. Thereafter your contract for the surgery will be with the SurGal Clinic.
Before you pay an acceptance and administration fee and before your surgery is confirmed, and a contract comes into existence, we will require you to sign a copy of our Business Terms and Conditions, which will either be sent to you electronically or in paper if preferred.
The booking is not accepted until the surgeon has accepted you for surgery on the first initial introduction from us via a medical questionnaire from yourself and a date has been secured. We will then invoice you. The payment of a £500 acceptance and administration fee then secures your procedure. It is only then that a contract exists.
Pricing Info
The price agreed will include your surgery, transfers on arrival from the airport in Brno or Prague to your accommodation or SurGal Clinic and the reverse transfer for departure, consultation with the surgeon, any pre-operative tests the surgeon requires, accommodation at SurGal Clinic, (as invoiced) pending on surgery requirements and accommodation and meals for the duration of your stay at SurGal Clinic.
You are liable for any other costs that you incur during your stay in the Czech Republic outside of SurGal Clinic. Hotel accommodation before or after surgery, use of telephone, services, meals and drinks at the hotel of your choice are additional costs which will be borne by you.
Payment Terms
Upon an agreement being made for a booking, an acceptance and administration fee of £500 of the total cost per person is payable. This is non-refundable. The 14-day statutory cooling off period applies. You are confirming that you understand the terms and conditions when you pay the acceptance and administration fee.
When the medical questionnaire has been signed or filled in online, you are confirming that you have understood and accepted these Terms & Conditions and have answered the questions correctly without leaving any important information out that could affect your surgery.
After your booking has been confirmed by your surgeon you will then be required to book your flights and forward the airline flight confirmation to Ivy Health, this will secure your surgery space. Once we have received this Ivy Health with then invoice you for the acceptance and administration fee of £500.
This is due immediately. When Ivy Health have received the acceptance and administration fee an amended invoice will follow showing the balance that will need to be paid to the SurGal Clinic on arrival in the Czech Republic.
You will then receive further details regarding SurGal Clinic address, contact numbers, your procedure and transfer times. We reserve the right to cancel your booking, and cancellation charges may apply for short notice or last minute changes. If you decide when you get to the clinic you no longer wish to go ahead with your procedure and this is not down to any medical reason or your Surgeon has decided you are unfit for surgery you will occur charges for last minute cancelling. The surgeons time and theatre space that has been put aside and any pre-ops if any have been performed.
Your Procedure
SurGal clinic will provide you with pre-and post-operative aftercare instructions and advice. If you do not follow this advice and there is loss, injury or damage arising from your lack of care Ivy Health cannot be held responsible.
Ivy Health do not accept any responsibility in the unlikely event that the surgeon feels that you are not fit enough to undergo surgery after pre-ops has been completed and Ivy Health will not offer compensation for expenses incurred. In this unlikely event the cost of any pre-operative procedures will be charged to you the client by Surgal Clinic.
Ivy Health hold no medical qualifications. We are not liable for the work of the surgeon or any of the staff engaged at your clinic. Your contract is with the clinic for all medical work undertaken whilst you are at Surgal Brno. Ivy Health Surgery Limited trading as Ivy Health Surgery accepts no liability for loss or injury of any kind arising from your lack of care.
Medical Questionnaire
You agree that your medical questionnaire will be forwarded by e-mail to the clinic. Ivy Health are not liable if you have not completed the questionnaire thoroughly and honestly or do not disclose to the medical staff at the clinic your full medical history.
We ensure that any information given to us is treated in a highly confidential manner and will only be sent to the clinic. We will not disclose any information regarding you without your written permission. By ticking the final Digital box on on the Medical Questionnaire you are agreeing all your Information is correct and and you have fully read understood out T&C prior to booking.
Travel Info
After we have received your deposit, you will receive any additional information relevant to you. Travel arrangements between your home and the airport are your responsibility. We recommend you have a suitable in date travel insurance policy that covers you and your property for the duration of your flights and stay in the Czech Republic.
Clients are responsible for the provision of all necessary valid documents. All UK nationals will require a valid full passport for travel, valid for at least 6 months before expiry. No visas are required by UK nationals at the time of going to press.
Please ensure that you check flight timings carefully. We can only advise on the requirements for British passport holders. Other passport holders and for British passports endorsed in any other way, enquiries should be made at the appropriate Embassy.
- British passport holders do not require a visa for entry into the Czech Republic.
Change of Booking
If you decide to change your booking, we must be notified of the decision immediately.
Cancellation of Booking
If you decide to cancel your booking, we must be notified of the decision in writing. Refunds? Notification period. Re-booking?
Circumstances
Except where we say differently elsewhere in these conditions, we cannot pay any compensation, expenses, losses or any other amount or otherwise accept responsibility if we must change your booking after departure or if Ivy Health or SurGal Clinic cannot provide your procedure as agreed, or you suffer any loss or damage of any description because of circumstances beyond our control.
When we refer to circumstances beyond our control, we mean any event which we or the SurGal Clinic in question could not foresee or avoid even after taking all reasonable care. Such circumstances will usually include war, terrorist activity, civil unrest, industrial dispute and bad weather (actual or threatened).
